Music Release Form
In signing this form, I acknowledge that I have been issued a music folder from the NU Symphony Orchestra. I understand that I am responsible for bringing it to every rehearsal and for returning it at the last concert of the semester to Nicole Liaw. Having music for each rehearsal is crucial to maximizing the learning experience for the whole ensemble and I understand that my failure to bring the folder or to practice my part will be reflected in my final grade for the class.
If I lose the folder, it is my responsibility to notify nuorchestra@gmail.com ASAP so that a new folder can be prepared for rehearsal. If I lose or fail to return the folder at the last concert, I will be responsible for paying $10 to the NUSO.
